Default Online Forms

I'm interested in setting up an auto-response in Outlook that allows me to
capture information from the sender. I would like to be able to have them
complete a short online form and then send it back to me of course, without
the form being sent to them again.

If that makes any sense, please let me know if you have any answers. Thank
you.

I'm afraid that doesn't make any sense, given the limitations of Outlook forms. You'd be better off providing a link to a Web page form.
