Default How to deploy a form created in Visual Basic Editor of outlook

Hi People,

I have created a from in Visual Basic Editor of Outlook. I don't know how
tho deploy that form on other machines. Can someone help me in this?

Also I want to open this form on click of some custom button on toolbar or
some custom menu, so please help me on this too.
