Default How to include template in reply

I am looking to set up that the user receives a templated email and when they
hit reply they are prompted to fill out the information needed in a form and
returned to the sender

Is this possible. I am looking to replace attaching a doc that they have to
fill out and reattach

A user inside your organization or out? If inside, using Exchange as the mail server or something else? If Exchange, do you have permission to publish a form to the server's Organizational Forms library? If not, then stick with the document attachment. If so, then you can consider using an Outlook custom form.
