Default Export Form Data into MS Access

I have done some VBA in Excel and Access, but I have never created a form in Outlook and exported the data into Access.

Can anyone give me a simple example that shows me how to do this in Outlook. (I'd love to see an example that isn't dealing with your contacts, because I have found a couple like that but its confusing me since the form is already in outlook.

Also how do you call your Code or Macros in Outlook?
