Default Printing Custom Fields from Outlook 2007

I have created a database in Outlook 2007 with a custom form, utilizing user
fields 1-4 from the miscellaneous fields section. However, when I go to
print an individual contact, those fields will not print (memo style). When
I try and edit the print style, it will only let me manipulate the font,
margin, paper and header/footer settings. It will not let me alter or edit
which fields it pulls to print.

Is there any way to include custom fields when printing? Thank you in
advance for your time and consideration.
