Default Opening individual contacts

Just got Outlook 2003 (from Outlook 2000). When I open a contact to open,
sometimes it give me an error window saying custom form not available,
outlook will open with outlook. Contact administrator. However, some of the
contacts open fine and the "form" is the same. New contacts aren't an issue.
Any thoughts on how to avoid that pop up message.

Thanks.

Scott W.
