Default Message and attachments not in a linked form

Hi

I have spent ages looking through the forum so far and found similar problems but not the same as mine

I have created (for the first time ever) three custom forms in OL2003

The 1st form is a request form that the user completes and attaches a document, they have to send this to an authorising manager. The only way i could find, to attach a document is to insert a 'message' area from the Fequently-used fields

When the mail has been received by the authoriser, the readers page has a approve/reject voting buttons which will link as a responce to either a Rejected form or an Approved form

the approved and the rejected forms remembers all the details of the user defined fields but when you send the form on to the next person to do the work it has lost all the information that was in the original Message area

i have tried changing the 'When Responding' on the Actions tab to attach original mail and include original message text. and for the 'Address form like' to Forward and numerous combination there of

i am assuming that i can include any free type text as a user defined text box rather than the message area, but im completely lost as to how i can get the attachment to copy from the 'Request' form to the 'Approved' form or the 'Rejected' form
