Default Creating toolbar button for form

I created a form and wrote a macro to make a toolbar button, but my toolbar
button does not work. However, I now have a link to and can access the form
by clicking "Actions". Did I do something incorrectly when I wrote the code?

This is the code I used:
Sub DisplayForm()
Set myFolder = Session.GetDefaultFolder(olFolderInbox)
Set myItem = myFolder.Items.Add("IPM.Note.MyForm")
myItem.Display
End Sub

I would appreciate any guidance.
