Default Contacts are slow to open with Outlook 2000

User has created a custom form for their contacts. When the user tries to
open a contact, it takes 2-3 minutes to open, sometimes locking up the
computer forcing us to 'end the task'. How do we go about changing the
Contacts form back to "Default" to see if the custom form is causing the
issue?
