Default Outlook form error message

Hi

I recently completed a custom form which was successfully tested. It
was published to the Exchange Server under a test name.

Upon passing user tests it was renamed to a live name. It now
generates an error "The form you selected cannot be displayed".
However, this only occurs when replying to the form, i.e. a user opens
the form, fills in some details and sends it with no problems, but when
the recipient clicks "Reply" the error message appears.

This did not occur until the name was changed. I have tried
republishing the form but the error persists.

I am at a loss as nothing within the form was changed.

Can anyone help?

Default Outlook form error message

Sounds like the Reply action on the (Actions) page is set to use the same original form, but the action was not updated to the live name.
