Default Storing data collected from email forms in a database

I would like to create a form (questionaire) in outlook. When the recipients
respond to the questionaire, I would like the information to automatically be
stored in a datebase (Access).

Before I do more research I would first like to know if this is possible.

Yes, by writing code to read the property values from Outlook and then put the data in the database. See http://www.outlookcode.com/d/database.htm

Note, though, that a questionnaire created as a custom form in Outlook is practical only if you are sending it to people in the same Exchange organization, are 100% sure they all use Outlook (not OWA), and have published it to the Organizational Forms library.
