How to create a re-usable form-template in Outlook. want to keep it in a
public folder and retrieve-complete it as needed- and email to another dept. for fulfillment |

That won't quite work. Either the form would need to published in the Organizational Forms library, not to the folder, or the completed items would need to be kept in the folder, with the department doing the fulfillment picking them up from the folder.
