Default editing the contact card template

I would like to add a field to the contact card. I need a tick box at the bottom of the card. The idea is everytime I click on new contact card, the tick box will be present in the template.

Default editing the contact card template

The information at http://www.outlookcode.com/article.aspx?id=21 explains why
you might not want to edit the main page of the contact form. You could
simply add a field to the folder and have it appear on the All Fields page,
without the need for a new custom form.

But if you do decide to customize the contact form, that same site has
details on how controls work and how to make your form the default.
