Default Creating Fields in Business Contacts

I need to create fields in the business contacts that will print even when
the field is not populated, so when I print the contact I can fill out those
fields with the correct information.

If I print now, the fields that are not populated do not print. e.g. if
there is no home phone entered in the home phone field, when I print the
field home phone will not print.
