What does it mean when a reply to an email has this: [!! SPAM] in the
subject line of the original email that had been sent?

This hint has been added by a spam tool that has identified this mail as
SPAM. Outlook's own Junk-Mail-Filter does not modify the subject or the
mail body of an spam email. You should be able to disable adding this
hint in the options of the tool you're using for malware protection.

This means anti-spam software running on either your machine or your mail
provider has scored the message high enough that it thinks it could be spam.
This functionality can be used in conjunction with an email rule in your
mail client to move all messages with [!!SPAM] in the subject line to
another folder for you to review.

If you are consistently having "good" messages scored this highly you should
be able to go into the spam filter settings and set the sender (or sender's
domain) as an allowed sender. Contact your software vendor (if it's
installed on your machine) or your email provider (if it is not) for
assistance in doing this.
