I downloaded outlook.maps.live so I can have the Location and mapping feature
for my calendar but after downloading, it doesn't see that it's there. It
keeps asking me to download and install it. I hate prurgatory!

Apparently this is the million-dollar question. I've gotten the install to
work, but there's no functionality once 2007 is restarted. Further, the
"form not found" error appears any time the calendar tools are used.

What's even more odd is that the add-in worked perfectly with the trial
version of Office 2007. Interesting that now it "breaks" the calendar in
Outlook 07 instead (cache operations are ineffective).

try a new profile - if it fails, uninstall the add-in, reboot and reinstall
it.
