That does not seem to be an option. I can select the first, second,
third, fourth, or last, but not the fifth. Am I missing something??

Nope. I'd say that adding 'fifth' to that drop down is such an edge case
that it wasn't considered worth adding and testing. I could imagine it
could be done in code, but it wouldn't be a recurring appointment, it
would just be individual appointments.
