That's apparently one of many things lost when the first page is customized. I've added it to the list at http://www.outlookcode.com/article.aspx?id=21. FWIW, the user can still modify E-mail 2 Display Name and E-mail 3 Display Name on the All Fields page.

Thanks for your speedy response - I thought I had broken a link
somewhere and I was tearing my hair out! Do you know if this has been
fixed in Office/Outlook 2007? I know there's another bug - when you drag
your custom form onto the "New Message to Contact", it does not open up
a new email - has that been fixed too? Just curious - thanks for all
your ongoing help - I'm sure many many people are grateful for your
assistance - I know I am!!
