Sorry - i forgot to mention the outlook form has a password set when you try
to design the page but when the custom form page went missing, there wasn't
any popup requesting for the password...

Hi,

I've created a custom outlook form abt a yr ago and it works fine until
recently when users state when they open up the form - they could only see
the message page and not the form anymore....
