Outlook 2003 IPM.NOTE (Form) Exporting to Access or Excel

I have been searching and trying to find a solution in order to pull
that data in this custom form to either access or excel. Can anybody
point me to the right direction?

I have seen information on how to pull contact forms out, but this is
not a contact form (unless I am confused)
