Looking to create an email form (a survey) in Outlook 2000, is there any
templates available for Outlook 2000 or is there an easy way to create the
form itself?

What is your scenario and mail environment? There are few circumstances where an Outlook form is a good solution for a survey.

I work for a mutual fund company and we're trying to gain feedback on our
website and we'd like to do this through an emailed survey form.

Outlook custom forms are definitely not suitable for that purpose. Why not put the survey on the web site?
