Default preset addressee

It is possible to create an outlook, or word, form that will go to a preset
addressee. I am trying to create a form that can have four fields filled in
by users and the address already set so it can be sent with one click.

Default preset addressee

Can you answer Yes to all these questions:

1) Are these users all within your organization?
2) Does your organization use Exchange as its mail server?
3) Do you have permission to publish a form to the Organizational Forms library on the server?

If so, then see http://www.outlookcode.com/d/forms.htm for basics on form design. If not, then an Outlook form is not a suitable solution to this scenario? If you have Office 2007, you can build a good survey email form with Access+Outlook.
