Default Generic Help Desk form from Microsoft site

I am trying to publish the Help Desk form Microsoft provides and have
modified only labels. I want to direct it to a mail-enabled public folder but
cannot find where to make this change. Can someone tell me how to do this? I
get this error: "Object required:
'CorrespondenceItem.userproperties.find(...)'.

I am not a programmer so this is probably a very easy thing to find but I
just am not finding it. Also, I don't see where I can enter a subject to this
form. Has anyone used this form just "out of the box" and gotten it to work?
Can you help a "newbie"?

What statement raises that error? Is the Microsoft form a message form or a post form? If it's a post form, you'll need to duplicate the form's functionality starting with a blank message form.

If it's a message form, then set the To address before publishing.
