Default Created form, sent, but addressee doesn't see fields & can't reply

I created a form to collect employee info, sent it, but the addressee doesn't
see the fields - only the text box. So obviously they cannot fill in a field
or reply, etc. I'm trying to collect data via this Outlook form. I do not
need it to behave as a survey (collect data automatically) as I will do this
manually. I just want someone to be able to view the form, fill it in, and
return to me -- in the same format it was sent. I am able to post this in an
organizational forms folder but that doesn't seem to help.

Did you leave the "send form definition with item" box unchecked? Create a read layout?
