When I import .csv file into Contact in outlook, it opens up the standard
Contact form. If I have created a custom form, and set "when posting to this folder use "new" form", how do I import contact from .csv file into the custom form? |

Outlook doesn't support importing to a custom form. You'd have to write custom code or use a third-party application. See http://www.outlookcode.com/d/customimport.htm.
