Questionnaire within Outlook
Hi,
Is it possible to create a questionnaire within the body of an Outlook e-mail message? I have googled and havne't come up with a conclusive answer on how this can be done. I have 5 questions (all required as Radio Button choices) which have 4 possible responses (Strongly Agree, Agree, Disagree and Strongly Disagree). If the user chooses Disagree or Strongly Disagree they should be prompted to complete a text box with comments. I have Outlook 2010 but will be sending to stakeholders that are using Outlook 2003 and 2007 on different Exchange Servers. Ideally I want to save the responses into a database where I can export to Excel but for now any advice on how I create a simple questionnaire within the body of an e-mail with the requested attributes would be very much appreciated.
