Assuming you know your account name and password then try putting
mail.nctv.com in the box for both? If your email address is karen at
nctv.com then mail.nctv.com has a chance of being right... it doesn't have
to be but it's quite common... so nctv.com is a clue if that is what your
email address is like.

Some ISPs need you to put in the full account name, others only the first
part... so if you email is karen at nctv.com try both combinations. (I wrote
at instead of @ to stop it forming links).
